ON Tuesday, July 10,2012 Samuel Augustus James McCormack passed away at at the Stollery Children's Hospital, Edmonton, Alberta at the age of 17 months Sam is lovingly remembered by His parents Noel and Ann McCormack of Clandonald Sister Louise and brother Jack Chosen grandparents Joan and Chris Logan of Cochrane Auntie Susan (Derrick) Rathbone of Spruce Grove Uncle Edward (Jade) McCormack of Clandonald Uncle Brendan McCormack of Clandonald Uncle Andy Marshall of Calgary and numerous cousins Sam was predeceased by His paternal grandparents Jim and Margaret McCormack and maternal grandmother Ann Marshall
